Willowmere Care Centre provides specialist neurological rehabilitation and dementia care placements for adults living with complex clinical needs across Barnet and the wider North London area. We work closely with hospitals, integrated care boards, case managers and community professionals to support individuals who require structured post-acute rehabilitation or longer-term specialist residential care within a safe and supportive environment.

Our service supports both step-down rehabilitation pathways and longer-term placements where individuals require ongoing specialist neurological or dementia care. We aim to make the referral process clear, responsive and collaborative so that each placement supports safe transitions, continuity of care and positive outcomes for the individual.

Making a Referral

Referrals to Willowmere Care Centre begin with a discussion with our team so that we can understand the individual’s clinical presentation, current support needs and placement objectives. Following receipt of clinical documentation, a pre-admission assessment is arranged to ensure the service can safely meet the individual’s needs.

Once a placement is agreed, we work closely with referrers, families and commissioning teams to support a structured and well-planned transition into the home. Communication continues following admission so that professionals remain informed and involved in ongoing care planning where appropriate.

Post-Acute Neurological Rehabilitation Placements

Willowmere supports individuals who are clinically ready to leave acute hospital or inpatient neurorehabilitation services but continue to require structured support within a residential setting. Placements are appropriate where ongoing rehabilitation, behavioural support or medical monitoring remain necessary to enable further recovery and pathway progression.

Our team works alongside referrers to support realistic goal-setting and continued therapeutic engagement within a safe environment that promotes independence wherever possible.

Acquired and Traumatic Brain Injury Support

We provide specialist residential placements for individuals living with acquired or traumatic brain injury who require continued neurological support beyond the acute stage of recovery. Placements are suitable where individuals present with cognitive, behavioural or physical changes that benefit from consistent staffing, structured routines and responsive care planning.

Support is designed to maintain progress already achieved within hospital services while providing stability within a longer-term pathway where required.

Huntington’s Disease Care Placements

We provide specialist residential placements for individuals living with Huntington’s disease whose neurological and behavioural needs require a coordinated multidisciplinary approach. Support is adapted as needs progress, ensuring residents continue to benefit from consistent care planning that promotes comfort, safety and long-term wellbeing.

Dementia Care

Willowmere Care Centre supports adults under the age of sixty-five living with early-onset dementia whose needs are not always appropriately met within traditional older adult dementia services. Placements focus on maintaining engagement, supporting communication and providing structure within an environment designed to meet the needs of younger residents.

Care programmes are adapted to reflect changing cognitive needs while maintaining meaningful daily routines.

Willowmere supports adults living with acquired brain injury, traumatic brain injury, early-onset dementia, Huntington’s disease and complex or advanced dementia who require specialist residential care or post-acute neurorehabilitation within a structured environment.

Yes. Willowmere Care Centre regularly supports individuals transitioning from acute hospitals and neurorehabilitation services who require continued structured support following discharge.

Yes. While some residents are working towards greater independence following rehabilitation, others benefit from longer-term specialist residential care. Care pathways are developed around each individual’s needs and goals.

Our team aims to review referrals promptly and will arrange assessments as quickly as possible once clinical information has been received. We work closely with hospital discharge teams and commissioners to support timely placement planning.
